Does the USS Enterprise's survivor roll occur immediately upon destruction, at the end of that players surface attack, or at the end of the surface attack phase?

The rules on pages 25;;34 and 35 pretty well tell you how and when to place damage counters and timing of damage effects; But some SA's especially with Bonus Hull Points need to be resolved instantly; Survivor-- Near Miss-- Torpedo Survivor--- Tenacity; so on Since you can roll against a ship multiple times and the SA can alter the attack results--the attacker needs to know before the end of the phase. This SA is also not used against Torpedo attacks and if already crippled.

When a ship comes back to life with survivor, near miss, et al it does not come back with the effects of crippled. Correct?
Because it never got "vitaled", and it only took 1 hit ... it's not crippled. However, if a ship survives with only 1 hit point ... then it is crippled.

As swizzle noted; no; not until you hit it enough to cripple it. Hence with Survivor you could possibly vital Enterprise 3 times in a turn before crippling her;rolll those lucky 5's; If you do it a fourth time no need to roll for Survivor last Hull point is now gone. Highly unlikely just an example on why you have to know damage with SA's as this, and apply it before the end of the phase.
