Does anyone know where I can find the "Scenario Template"?

Many of the scenarios that I've "rescued" from the Forumini where beautifully converted by Sturmvogel to look like the format that WOTC used for it's official scenarios.  Question is does anyone know how to obtain the template that Sturmvogel used?  I could have sworn he had shared the pdf, but I couldn't find it.  It would be a great resource to have on this Forum (if that's okay with Sturmvogel), and another Foruminian has asked me for it also.
